We require the services of a Community Social Worker that will work collaboratively with St Luke’s Combined Hospice’s Inter-Disciplinary Team (IDT) to provide psychosocial support to patients, their family members and significant others.
ENTRY-LEVEL QUALIFICATION, KNOWLEDGE AND EXPERIENCE
- Professional Social Work qualification. (BA Social Work, BSocSci, Diploma in Social Work)
- A minimum of 3 year’s related training and/or related experience in a similar position.
- Previous experience in a Health care setting
- Experience in palliative care and understanding its role in society will be an added advantage
- Certification in Palliative Care (added advantage). Those applicants who do not meet this criterion, will be expected to study towards the certification
- Current registration with the SACSSP
- Fluency in English and 1 other official language
- Intermediate MS Office Suite literacy – Outlook, Word and Excel.
- Unendorsed driver’s license with long-term experience driving a manual vehicle.
KEY PERFORMANCE AREAS
- Provide holistic psychosocial assessment of patients
- Provide counselling support to patients and families
- Facilitate material support for food security ,grant applications, wills, UIF and other psychosocial support as required
- Perform crisis interventions when necessary
- Compile and submit reports e.g. Psycho-Social Assessments and patient Statistics
- Refer patients to other specialised services within the organisation when so required
- Maintain all patient data in accordance with the requirements and guidelines of SLCH
- Work in an Interdisciplinary team approach to render intervention to patients and families
- Attend meetings and workshops for continuous professional development. Perform other duties, as and when so requested, in support of team goals
